	Please list any business sharing your space or advertising in your booth: NOTE: All business participating or advertising in the show must fill out individual applications and pay the minimal $50.00 fee, even if they share a space with another business.

	Please list the items (if any) you will have available for give-away or sampling by the consumers: NOTE: The following items MAY NOT BE SOLD but FREE samplings may be offered to promote the product - prepared meats, sandwiches, candy, chips and beverages.

	Release of Responsibility I have read and fully understand all rules pertaining to the Ham Days Festival Civic Row Area and agree to abide by them completely. I further understand that Chamber of Commerce or any of its entities are not responsible for lost good due to theft, weather or any other unforeseen incident of loss. I also understand that if I do not comply with the associated rules, I will be asked to leave with no refund. I accept this release of responsibility. (required)
